The Contribution (or Gifts to the District) form is used for donations made to the school(s) or the district. This form should be used for all goods and services and monetary donations made to the school(s) or district including in-kind donations or services.
High School: If you do not wish to have your student's information shared with the military, please complete the district Opt-Out form and return to your school's office no later than October 1st of the school year. Once completed, the form will remain on file during your student's high school career in the Marysville School District.
