Clock Hour Proposal Form

  • Proposal must be submitted for approval prior to actual training.

  • Training approved for clock hours must be a minimum of 1 hour and are only done in whole hour increments.

  • Clock hour approval cannot be granted for routine staff meetings.

  • Each training approved for clock hours will be evaluated by participants

This form is utilized to receive permission for parent or guardian to photograph a minor child for school and district publication purposes.

Consent to Photograph - Classroom Based AssessmentsThis form is used to obtain parent / guardian permission to photograph, videotape or audio record a minor child for Classroom Based Assessments work.

Field Trips

Field Trip Handbook
This Field Trip and Outdoor Education Handbook provides guidelines to ensure the safety of all students, staff and volunteers and to help reduce the district's liabilities by effectively eliminating and/or reducing loss exposures and risks. This handbook also includes required forms.


Field Trip Permission Slip
General permission slip for participation in field trip activity.\


Field Trip Permission Slip - Secondary
Secondary school related absences - permission slip for participation in field trip activity. This form requires both parent/guardian and teacher signatures.


Field Trip Health Information Form
Overnight field trip health information to be completed by parent/guardian.


Field Trip Medication Form
Overnight field trip medication request form for elementary and secondary students to be completed by parent/guardian.


Field Trip Office Notification and Pre-Approval Form Field Trip Notification Form must be submitted to Principal for pre-approval. Once approved, the information will be given to Secretary for processing. This is a fillable form.

Procedures for Calling 9-1-1 on a Field Trip. This fillable form should be completed prior to field trips.

Submitting a grant or project? Staff must complete this Application Request Form (fillable form). Please review the Guidelines prior to completing. 1) Since this is a PDF fillable form, please save to your Desktop or My Documents prior to completing, or 2) You may also print and fill it out.


Classroom Material Usage

Notification of Video Film Usage
Please refer to Policy 2020P: Course Design, Selection and Adoption of Instructional Materials. Staff may utilize films, videos and other multi-media so long as such use is consistent with th District Audio/Visual/Electronic Material Approval Guidelines outlined in Appendix F.


Course Proposal or Modification 
Please refer to Policy 2020P: Course Design, Selection and Adoption of Instructional Materials. Appendix A is for Course Proposal. Proposals for a new or restored program will be submitted by the principal of a building, or director responsible to the Learning and Teaching Department by the first Monday of April of the preceding year.


Request for Course or Material Adoption
Please refer to Policy 2020P: Course Design, Selection and Adoption of Instructional Materials. Appendix B must be completed and will be used to evaluate instructional materials.


Guest Speakers

Guest Speaker Request Form
Please refer to Policy 2020P: Course Design, Selection and Adoption of Instructional Materials. Appendix G must be completed and all guest speakers must be approved by the principal in advance of their presentation.
